18. What are the first signs of liver problems, and what symptoms should you pay attention to first?

Constant heaviness in the right side, flatulence, discomfort in the abdomen, bitterness in the mouth after sleep - most often this is the first sign of “liver” problems. The relevance of such a problem as alcoholic liver disease is very great. In terms of prevalence and social significance, this pathology ranks second after viral hepatitis.

There are no safe doses of alcohol. According to WHO recommendations, up to 20–40 ml of ethanol for men and up to 20 ml of ethanol for women can be considered relatively safe doses of alcohol. A dose of 10 ml of ethanol is contained in 25 ml of vodka, 100 ml of wine or 200 ml of beer.

The myth about the harmlessness of “weak” alcohol

There is a common misconception that if you drink weak alcohol (beer, low-alcohol cocktails, etc.), then it will benefit the body as a whole. But is it?

The effect and harm of low-alcohol drinks is equivalent to the effects of strong ones. And the main reason for this equivalence is quantity. Few people think about how much ethanol enters the body if you drink “just” a few bottles of beer a day.

You only need to drink three bottles of beer or two cans of low-alcohol cocktail to get the same amount of ethanol as is contained in a glass of vodka.

Low-alcohol canned cocktails are also gaining popularity, especially among young people. Due to their sweetish taste, they are perceived as strong lemonade. Therefore, we can confidently conclude that beer and other low-alcohol drinks are no less harmful than strong alcohol.

Liver and alcohol

The effect of alcohol on the liver is direct. After entering the body, part of the alcohol is eliminated through the skin, lungs and kidneys. The main “blow” and harm (and this is about 90% of consumed alcohol) is taken by the liver, where further processing of the alcohol consumed occurs.

First, under the influence of a special enzyme - alcohol dehydrogenase - ethyl alcohol is oxidized and converted into acetaldehyde. Next, acetaldehyde is oxidized through complex chemical reactions and decomposes to the final substances - carbon dioxide and water. But this only happens when the amount of alcohol is small and sufficient to completely complete the cycles of converting ethanol into breakdown products that are not harmful to the body and do not cause poisoning.

If a lot of alcohol is consumed, enzyme deficiency occurs and the process of ethanol processing at different stages is disrupted. There is an accumulation of intermediate products of its oxidation and breakdown in the liver. Acetaldehyde is tens of times more toxic than ethanol; its excess causes, in addition to its direct damaging effect, disruption of the normal outflow of bile, accumulation of fats in the liver, and general poisoning of the body. And ethyl alcohol, which does not oxidize due to excess, causes the development of connective tissue in the liver, which leads to fibrosis.

The liver has amazing regenerative functions and the ability to heal itself. But these liver abilities are not endless, and, in the end, this potential is exhausted. Liver cells begin to be replaced by fibrous tissue with a restructuring of their functions and transition to cirrhosis.

What mechanisms and risk factors cause alcoholic liver damage?

First of all, this is chronic alcohol abuse. Sometimes the quantity and quality of liver enzymes that metabolize alcohol can be genetically impaired.

It has been noted that women are more susceptible to alcohol and their addiction develops faster.

When combining alcohol intake with drugs metabolized in the liver, the harmful effect on hepatocytes increases several times.

The lifestyle that a person drinks alcohol leads also matters. It is known that the shortage nutrients(malnutrition, dieting) and alcohol are incompatible.

If there is viral hepatitis, then the negative effect of alcohol on the liver increases with its consumption. Vaccination against hepatitis helps prevent the disease. A quarter of patients diagnosed with alcoholic liver disease have antibodies to chronic hepatitis C, indicating an increased risk of infection with this type of virus.

Vaccination against hepatitis - for or against?

In recent years, distorted opinions have been formed in society related to the alleged harm of vaccinations against dangerous diseases on the body. Therefore, I would like to separately touch on the topic of vaccination against hepatitis.

Currently, there are vaccinations against two types of viral hepatitis: A and B.

Hepatitis A is considered a “disease of unwashed hands”, the main route of transmission is household.
Hepatitis B is transmitted primarily through blood. To transmit the virus, a drop of blood that remains, for example, in a syringe needle is enough. But one should not think that this is a disease only of drug addicts or socially disadvantaged elements. The spread of hepatitis B has become an epidemic in recent years.

Hepatitis B in most cases becomes chronic, which can lead to the development of cirrhosis and even liver cancer.

Vaccination against hepatitis B is included in the vaccination schedule for children. Adults are given this vaccination if they wish and consent. Typically, the vaccination schedule looks like this: 0 – 1 – 6. That is, the vaccination should be repeated after 1 and 6 months.

Many people have questions about the effect of alcohol on the effectiveness of hepatitis vaccination. Drinking alcoholic beverages in acceptable doses has no effect on vaccination. But you will still need to stop drinking alcohol for three days after vaccination due to the fact that each vaccination has its own side effects, which alcohol intake can aggravate.

This is also true for vaccination against other pathogens. The exception is the rabies vaccination, after which it is recommended not to drink alcohol for 12 months.

Alcoholic liver disease

It develops as a result of long-term poisoning of the entire human body with alcohol (ethyl alcohol).

During its course, three successive stages of development are distinguished:

  1. Liver steatosis (fatty degeneration; fatty hepatosis);
  2. Alcoholic hepatitis;
  3. Cirrhosis of the liver.

Hepatosis (steatosis)

The initial stage, or fatty hepatosis, is characterized by the deposition and accumulation of fat cells in the liver parenchyma. Occurs in people who abuse alcohol in more than 90% of cases. It is usually asymptomatic, there may be symptoms of dyspeptic disorders, a feeling of heaviness in the right hypochondrium, weakness, yellowness of the skin and mucous membranes. Liver steatosis is a reversible process, and with abstinence from alcohol and treatment, complete recovery is possible. If further exposure to ethanol occurs on hepatocytes, alcoholic liver damage progresses to the second stage - alcoholic hepatitis. In this case, deeper damage to hepatocytes occurs with disruption of their functionality.

Alcoholic hepatitis

The course of alcoholic hepatitis can be in the form of a persistent (stable course, usually without or little symptoms, relatively reversible changes in the liver) or a progressive form (transition from the previous phase in case of further alcohol abuse; an unfavorable course, as a rule, turns into cirrhosis).

Hepatitis can occur with an acute onset or have a latent and then chronic course. The acute form of hepatitis is usually observed after a long, often repeated binge in an alcohol abuser, when the body is poisoned with large doses of alcohol.

There are several varieties of the course, but the most common is the icteric variant. In this case, in addition to jaundice, pain in the right hypochondrium, severe weakness, loss of appetite, nausea, vomiting, prolonged loose stools, and noticeable weight loss are observed. The liver is enlarged, has a dense structure, and is painful.
Chronic hepatitis is often relatively asymptomatic. Only laboratory and additional research methods can indicate severe damage to the organ. The liver also enlarges significantly, reaching enormous sizes.

The third stage is cirrhosis of the liver. This is a completely irreversible stage, when normal liver tissue is replaced by fibrous cords and connective tissue fibers and, as a result, a profound disruption of all its functions develops. Cirrhosis occurs, according to statistics, in 15–20% of patients with chronic alcoholism. Worsening of the symptoms and course of cirrhosis is observed when combined with chronic viral hepatitis B or C, obesity, and being female.

Symptoms of cirrhosis can be quite sparse, especially against the backdrop of a long course of the disease. The patient is concerned about fatigue, weakness, pain in the liver area, asthenia (weakness, fatigue). A characteristic “hepatic” erythema (redness) appears on the palms, and small capillaries expand over the entire surface of the body. The liver is enlarged or, conversely, reduced, may already be painless, its surface is bumpy. Signs of ascites and an enlarged spleen are revealed, as a result of which the abdomen becomes enlarged with an expanded venous network on its surface. Symptoms of portal hypertension appear. The functions of other organs and systems are disrupted as a result of poisoning by toxic products that are not neutralized by the liver.

The course of cirrhosis is unfavorable. The disease is combined with progressive liver failure, up to hepatic coma, which leads to death. There is also a high probability of malignancy - the occurrence of hepatocellular carcinoma.

The diagnosis is confirmed by laboratory, ultrasound or radioisotope studies, computed tomography. If necessary, diagnostic laparoscopy and liver biopsy are performed.

How is alcoholic liver disease treated?

The most important condition is the patient’s complete abstinence from alcohol. At some stages of alcoholic liver damage, complete regeneration of liver tissue can occur. But it should be noted that, unfortunately, no more than a third of patients follow this recommendation. Another percentage simply reduces the amount of alcohol they consume, while the rest completely ignores it and continues to lead their normal lifestyle.

The second condition is a complete energy diet with a high protein content. The calorie content of such a diet should be at least 2000–3000 kcal per day. Protein content is about 1 g per 1 kg of patient weight. It is necessary to be saturated with vitamins, especially group B. An example of such a diet would be table No. 5.

Drug treatment

Hepatoprotectors, glucocorticoids, agents that improve microcirculation and similar drugs are used. Of course, the necessary treatment in each case is prescribed by the doctor individually after a full examination and taking into account the identified disorders.

If conservative treatment is ineffective, the issue of liver transplantation is decided.

If we talk seriously about the symptoms that clearly indicate liver disease, these are jaundice, skin itching, baldness, redness of the palms, and in women - disorder menstrual cycle; heaviness in the abdomen, nausea, belching, unstable stool.

In the later stages, there may be an enlargement of the abdomen due to the accumulation of fluid in it (ascites), darkening of the urine (it becomes the color of beer), discoloration of stool, and subcutaneous bleeding. In order not to lead to such a state, you need to take care of yourself at the first unfavorable symptoms. In this sense, the liver is a grateful organ - with proper care, it recovers well, and healthy cells can work for themselves and their injured comrades.

What happens with cirrhosis? A healthy liver consists of hepatic lobules. With cirrhosis, a change in the structure of the liver occurs; the liver contains a large number of connective tissue, and instead of lobules, nodules of different sizes appear, the overgrown scar tissue compresses the blood vessels, as a result, blood circulation is impaired. The loss of normal liver cells leads to the fact that the liver is unable to synthesize proteins and other substances needed by the body, as well as neutralize toxins, that is, to perform its functions.

What will help to make a correct diagnosis? First of all, a visit to a gastroenterologist, who, in addition to a visual examination, can use the following diagnostic methods:

  • a biochemical blood test that will show an increase in liver enzymes and an increase in the amount of bilirubin;
  • Ultrasound of the liver and other abdominal organs will reveal changes in the size and structure of the liver, the presence of fluid in the abdominal cavity;

  • examination of liver vessels (angiography, splenoportocavagraphy) using a special contrast agent under X-ray control is necessary when planning surgical treatment.

It's never too late to quit drinking

THE MOST common reasons liver cirrhosis - alcohol abuse and viral hepatitis (primarily hepatitis B and C). Less commonly, they include poisoning with toxic substances (for example, toadstool venom, certain medications), diseases of the gallbladder and bile ducts, accompanied by prolonged jaundice.

Increased pollution environment, increased consumption of various preservatives, alcohol, and uncontrolled use of medications increase the load on the liver.

According to experts, the next 10 years will be marked by an increase in chronic liver pathology associated with the hepatitis C virus.

As for the effect of alcohol on the liver, if we talk about “normal” drinkers and high-quality alcoholic beverages, the time that must pass from the start of consumption until the development of liver disease depends on many factors. Firstly, it depends on how much a person drinks, and secondly, who drinks - a man or a woman. In a woman with others equal conditions the disease will develop faster and at lower doses. It is believed that for men the maximum permissible dose is 60 g of pure alcohol per day: this is 3 bottles of beer, or 3 glasses of dry wine, or 3 glasses of vodka. The liver cannot process anything beyond this without causing damage to itself. In women, liver damage can develop even with constant intake of 20 g of alcohol per day.

On average, at least 8-10 years pass from the beginning of systematic drunkenness to the appearance of classic alcoholic cirrhosis. In women and adolescents, this period may be shorter - about 5 years. Alcoholic liver damage for a long time may be asymptomatic. A person drinks and drinks until the liver is no longer able to pump blood through itself, stagnation occurs in the portal vein system, increased pressure in it and, as a result, ascites - accumulation of fluid in the abdomen. Often, patients come to the doctor for the first time only at this stage - the stomach has begun to enlarge due to the fact that fluid has appeared there. This is a very important symptom and a serious prognostic sign. Often, if ascites appears, the patient’s life expectancy is several years. If a person does not stop drinking, this gap will rapidly shrink. However, if the patient can be convinced to give up alcohol, then despite the fact that he came to the doctors already at the stage of ascites, they can observe him for 10 or more years without further deterioration. Why? Because he stops drinking, there is no progression of the cirrhotic process, and restoration of liver function under these conditions may well occur. Therefore, it is never too late to quit drinking alcohol.

How to protect your liver?

There are quite a lot of MEDICINES on the market. Some of them stimulate the production of essential phospholipids - substances necessary to restore the integrity of liver tissue. Others already contain essential phospholipids in ready-made form, of natural origin, easily absorbed by the body.

Even if the process in the liver ends with recovery, the damage still does not go away without a trace and can make itself felt by a certain functional inferiority, one or another degree of hepatic dystrophy. To avoid this, your doctor may recommend hepatoprotectors - drugs that help the liver restore its structure and protect it from adverse influences. Now there are about 30 types of hepatoprotectors on our pharmaceutical market: herbal, homeopathic, synthetic. Most drugs plant origin, including homeopathic ones. Doctors prefer to prescribe them because these drugs have a wide spectrum of action, minimal amount side effects and reasonable price.

Treatment is necessary because the complications of liver cirrhosis are very serious. Hepatic encephalopathy is damage to the brain from toxic products that are not neutralized by the liver as a result of its damage.   • Various disturbances of consciousness and behavior (decreased concentration, daytime sleepiness, disrupted night sleep, etc.).
  • Decreased appetite and abdominal discomfort (bloating, feeling quickly full while eating).
  • Jaundice (yellow coloration of the skin and sclera).
  • Lightening or discoloration of stool, darkening of urine.
  • Stomach ache.
  • Swelling of the legs and (or) an increase in the size of the abdomen due to free fluid in the abdominal cavity (ascites).
  • Bleeding: nasal, gastrointestinal, gum, hemorrhoidal, and subcutaneous hemorrhage.
  • Frequent bacterial infections (respiratory tract, etc.).
  • Decreased sexual desire.
  • In men, gynecomastia (enlarged mammary glands) is common.

    “To drink or not to drink?” - Many of us ask ourselves this question. However, there is still no consensus on the effect of alcohol on health in the scientific community.     Nobody argues with the fact that in large doses alcohol does nothing but harm. Doctors even say that there is not a single organ on which alcohol does not have a bad effect.     However, most often liver diseases (both viral and alcoholic) remain silent for decades. Last year, only 45 liver transplants were performed in the city. For comparison, in America, 800-900 are performed annually in just one center,” continues our expert.

    But the worst thing is that liver diseases are often missed even by doctors. The fact is that it is difficult to diagnose liver pathology using ultrasound, and biochemical blood tests do not always show that the destruction process has begun. “The other day, a 32-year-old woman came to us; she was diagnosed with cirrhosis, the stage is close to terminal. Before coming to us for a consultation, she had not complained about her health, she worked, was an exemplary mother, but cirrhosis develops over decades! Now only a transplant can help her. Another patient came in regarding a slight increase in blood biochemical parameters. And she has complete liver damage due to uncontrolled use of painkillers. A few days later we transferred her to intensive care with a hepatic coma. Such cases are not uncommon in our practice. Fortunately, she was lucky, Moscow doctors were able to give her a liver transplant,” admits Igor Gennadievich.

    The vast majority of cirrhosis that is diagnosed in Muscovites (and Russians as well) is caused by alcohol consumption. The growth curves of ethyl alcohol production in the country and the mortality rate of Russians, as they say, go hand in hand. “You can treat this as you like, but from 1985 to 1989, during the anti-alcohol campaign, life expectancy among men in our country increased by three years, and mortality from liver cirrhosis decreased threefold,” says Professor Bakulin.

    However, few who drink or drink occasionally think about the harm it causes to the liver. Moreover, many have heard about the existence of safe doses of alcohol - a glass of red wine or a glass of vodka a day. And although few people limit themselves to such doses, knowing about safe doses warms the soul.

    — There are no harmless doses of alcohol, this is a myth. And our doctors spoke about this back in 1915 at the Pirogov Congress. At the same time it was stated that alcohol does not cure any disease. And in our bookstores there are a lot of books from the series “Treatment with Vodka,” “Treatment with Moonshine,” “Treatment with Beer,” Igor Bakulin is sure. — The European Association for the Study of the Liver also states: there is no reason to assume the existence of safe doses of alcohol. But sometimes even cardiologists recommend their patients to take a glass “for blood vessels.”

    Some people believe that only adulterated or homemade alcohol can cause harm. Doctors cite an aphorism from Abraham Lincoln: “Alcoholism does not come from drinking bad vodka, but from abusing good vodka.” Igor Bakulin warns that the path from a romantic evening with a glass or two to the pathology table can be very short: “I now have two patients in the department, 25-year-old girls, with alcoholic cirrhosis of the liver in the terminal stage.”

    And yet, in the case of treating patients with alcoholism, doctors today set the goal of at least transferring them to small doses (foreign statistics say that half of alcoholics with liver problems are ready for only partial abstinence). This is up to 30 g of pure ethanol per day for men and up to 20 g for women. If you drink in such doses, only every third person will develop alcoholic hepatitis after 5 years. If you drink 80 g of pure ethanol per day, almost half of the cases will develop cirrhosis, and every tenth person will develop liver cancer.

    In general, doctors call doses of 40-80 g of pure ethanol toxic to the liver, which corresponds to 100-200 ml of vodka, 400-800 ml of dry wine or 800-1600 ml of beer. These doses are designed for men; for women they are half as much.

    - These are not safe doses, no. But alcohol in such quantities brings much fewer health risks. The study found that switching to low-dose alcohol consumption reduces the risk of developing coronary disease heart and the risk of mortality from it by 15%. By the way, the effect is comparable to some medications,” says the chief gastroenterologist of Moscow.

    Another study among 10 thousand people with alcohol dependence proved that reducing the days of heavy drinking from 23 to 11 per month reduces cases of illnesses and injuries by 6813, pancreatitis by 2016, strokes by 152, and cirrhosis by 873. What would have happened if these people didn’t drink at all!..

    “Even for those who drink alcohol in small quantities, I would advise you to think about your health and undergo liver diagnostics. In the early stages, even cirrhosis is reversible, and today there are methods to treat it, says Igor Bakulin.

    In order to encourage people who drink a little (or more) to undergo diagnostics, the “Check Your Liver” program has even been developed. In accordance with it, “Healthy Liver” rooms are opening in clinics in 60 Russian cities, including Moscow, where you can do an ultrasound and undergo a biochemical blood test. Thanks to this program, doctors hope that the detection of liver diseases will increase significantly. Another task, notes Igor Bakulin, is to introduce into mass medical practice modern methods diagnostics - elastometry (a study using a special parting) and fibrotests (high-tech blood tests).
