Mandatory reporting forms

You must report using the following forms:

“Information on the activities of an individual entrepreneur.” This form is filled out by all entrepreneurs whose activities are not related to agriculture. You must report by March 2 of the year following the reporting year.

“Information on the activities of an individual entrepreneur in retail trade.” This form must be used to report to entrepreneurs who are engaged in retail trade and provision of services to the population. You must report by October 17 of the year following the reporting year.
